for failure to sell or dispense any drug, medicine or poison in good faith, failure to conduct a proper drug utilization review before dispensing drugs to a patient, made a material misrepresentation in furnishing information to the Department, exhibited a pattern of conduct which demonstrates incompetence or unfitness to practice and engaged in dishonorable or unethical conduct of a character likely to deceive, defraud or harm the public.
