after the Department received information on or about April 10, 2013, that Respondent's Missouri Pharmacist License was voluntarily surrendered for diversion of controlled and uncontrolled substances from Family Care Pharmacy, St. Peters, Missouri. Respondent was subsequently excluded from participation in any capacity in Medicare, Medicaid, and all Federal health care programs.
