due to violations of an Agreement entered into with the Department of Financial and Professional Regulation, Division of Professional Regulation of the State of Illinois, on February 7, 2017, namely, failure to submit to drug testing, failure to obtain the services of an addictionist and failure to participate in the Illinois Health Professionals Health Program.
