pharmacist license has been placed on indefinite probation for a minimum of two years, must complete 20 hours of CE on the topics of proper dispensing, recordkeeping, pharmacy law, and ethics, and fined $20,000 and may no longer serve as pharmacist-in-charge of Oak Street Pharmacy after Respondent's DEA certificate of registration was suspended due to Respondent failing to dispense high dosage opioid prescriptions in good faith to three patients.
